Required Forms

Students in grades 6-12 who are interested in playing a sport should review the 2025-26 start dates and athletic form requirements. In order to be eligible to try out for a team at any level or perform any pre-season training, FHSAA Athletic Eligibility Forms must be completed, signed, and uploaded to AthleticClearance.com.

A delay in submitting these forms will prevent a student from participating in any sports activity.
