Required Forms

Students in grades 6-12 who are interested in playing a sport should review the 2026-27 start dates and athletic form requirements as soon as possible. In order to be eligible to try out for a team at any level or attend any practice, All required FHSAA Athletic Eligibility Forms must be completed, signed, dated, and uploaded into your account on AthleticClearance.com.

A delay in submitting any of these forms and certificates will prevent a student from participating in any sports activity.

Please note: You do NOT have to wait until the desired season/sport to start uploading your Forms. To avoid any delays in your athletic clearance process, which will affect your ability to participate, upload your forms right after completion.
